The theoretical appeal of restoring NAD+ levels is therefore understandable, even if the clinical application is still far ahead of the supporting evidence.

NAD injections are administered either intramuscularly (IM) or intravenously (IV). The IV route delivers NAD+ directly into the bloodstream; IM administration delivers it into muscle tissue, where it is absorbed into circulation more gradually. Proponents argue that injectable routes offer higher bioavailability than oral supplements, but human pharmacokinetic data on how injected NAD+ is distributed, metabolised, and how long any increase in cellular NAD+ levels actually persists is not well characterised. That matters when evaluating clinical claims.

The Proposed Benefits and What Evidence Exists#

The benefits marketed by private clinics fall into several broad categories. It is worth being honest about where the evidence sits for each.

Energy and Fatigue#

This is the most commonly cited reason people seek NAD injections. The logic is straightforward: NAD+ is central to mitochondrial ATP production, so supplementing it should improve cellular energy output and reduce fatigue. Some people report meaningful subjective improvements. The difficulty is that without placebo-controlled trials, it is impossible to separate genuine pharmacological effect from expectation. No large randomised controlled trials have established NAD injections as an evidence-based treatment for fatigue in humans.

Anti-Ageing and Cellular Repair#
