For this to work, the concentration of SNAC around the tablet needs to be high enough, and food, excess water, or other substances all dilute or disrupt that process.

This is not a minor inconvenience; it is the difference between the medication working well and barely working at all.

The Morning Routine: Step by Step#

Here is what taking oral Wegovy correctly looks like in practice:

  1. Wake up, before doing anything else including making coffee or having water
  2. Take one tablet out of the original bottle
  3. Swallow it whole with up to 120ml (roughly half a standard mug) of plain water
  4. Do not eat, drink anything other than a small sip of water, or take other oral medications for at least 30 minutes
  5. After 30 minutes, continue your morning as normal

That 30-minute wait is a minimum. Some pharmacokinetic data suggests that waiting up to two hours before eating improves absorption further, though the prescribing information specifies 30 minutes as the required threshold.

What You Can Do During the Wait#

This is one of the most common practical frustrations people raise. Thirty minutes without coffee, breakfast, or other tablets can feel disruptive if you have a busy morning. A few things that genuinely help:

  • Prepare a measured 120ml glass of water the night before and leave it on your nightstand so you can take the tablet immediately on waking, before you even get up
  • Use the 30-minute window for a shower, light stretching, or getting dressed
  • Set a phone alarm to remind you when the wait is over
  • If you take other morning medications, talk to your prescriber about timing; some can be shifted to later in the day

The one thing you cannot do during this window is have coffee. Even black coffee counts as a liquid other than plain water and will interfere with absorption.

Do not crush, split, chew, or dissolve the tablet. The pill's structure is part of how it works. Disrupting it will likely reduce effectiveness significantly, and there is no approved alternative formulation that allows this.

Storage: Keep It in the Original Bottle#

Oral semaglutide tablets are moisture-sensitive. The original bottle is designed to protect them from humidity, which is why you should not transfer them to a pill organiser or a loose container by the bedside. Keep the bottle sealed and store it at room temperature, away from direct sunlight and heat sources.

If you travel frequently or live somewhere very warm, be mindful of storage temperatures. Leaving the bottle in a hot car or a bathroom cabinet that gets steamy after showers is not ideal.

Drug Interactions: This Is Important#

Because oral semaglutide slows gastric emptying and is taken in conditions that affect absorption, it can interfere with how your body absorbs other oral medications.

The most studied interaction is with levothyroxine (the thyroid medication also known as Synthroid). Research in Expert Opinion on Drug Metabolism and Toxicology found that oral semaglutide can increase levothyroxine absorption if the two are taken together. This might sound like a minor effect, but for people with hypothyroidism who rely on precise levothyroxine dosing, it can shift thyroid levels meaningfully.

The practical solution is straightforward: wait at least 30 minutes after taking oral semaglutide before taking levothyroxine, and tell your prescriber you are on both medications so they can monitor your thyroid function accordingly.

Other medications that require careful timing include:

  • Statins (e.g. atorvastatin, simvastatin): timing adjustment may be needed
  • Blood pressure medications: particularly if taken on an empty stomach
  • Oral contraceptives: current evidence does not suggest a significant interaction, but discuss with your prescriber if you have concerns
  • Iron supplements or calcium tablets: these should generally be separated from oral semaglutide by at least 30 minutes

Dosing Schedule and Titration#

Skip it. Do not double up the next morning to compensate. Simply take your next dose as usual the following day. Missing an occasional dose will not significantly affect your overall progress, but taking a double dose increases the risk of side effects without providing additional benefit.

Managing Side Effects Alongside the Pill#

Nausea and digestive discomfort are the most commonly reported side effects with GLP-1 receptor agonists like semaglutide. With the oral form, some patients find these symptoms slightly different in character compared to the injection, partly because the medication is absorbed through the stomach rather than subcutaneous tissue.

Practical ways to reduce gastrointestinal side effects include:

  • Eating smaller, more frequent meals rather than large ones
  • Reducing fatty, fried, or heavily processed foods during the early weeks
  • Staying well hydrated throughout the day (just not in that first 30-minute window)
  • Avoiding alcohol, which can worsen nausea and also interacts poorly with the medication's effects on gastric emptying

A clinical consensus published in the Journal of Clinical Medicine recommends a high-protein, moderate-fibre dietary approach during GLP-1 therapy, both to reduce GI side effects and to support muscle preservation during weight loss.

If you vomit shortly after taking the tablet, do not take another one that day. Treat the missed dose as you would any other and resume the following morning.

Lifestyle Changes Still Matter#

Oral semaglutide is not a standalone solution. A 2025 meta-analysis in EClinicalMedicine (part of The Lancet) confirmed that patients who combined GLP-1 receptor agonist treatment with structured lifestyle changes lost significantly more weight and saw greater improvements in cardiometabolic markers than those relying on medication alone.

The UK Situation: Injectable Wegovy Now, Oral Semaglutide Coming#

In England, injectable Wegovy is available on the NHS through specialist weight management services (Tier 3 and Tier 4 services) for people who meet specific criteria, generally a BMI of 35 or above with at least one weight-related health condition, or a BMI of 40 or above. NICE approved Wegovy (injectable semaglutide) for weight management in 2023.
