If you miss a dose of Foundayo (orforglipron), take it as soon as you remember, as long as you haven't already taken today's tablet. If you're only realising now that you missed yesterday's dose too, just skip it and carry on with today's as normal. Never take two tablets in one day to catch up.

Unlike some medicines, Foundayo doesn't have a cut-off time for a missed dose. The UK patient information and prescribing leaflet both say the same thing: take it when you remember, provided today's dose hasn't already gone in. There's no need to check the clock, only the day.

Remembered late, or missed a whole day?#

Here's the quick version:

  • Same day, any time (even at bedtime): take it now. It still counts as that day's dose.
  • Next day, before your usual time: take today's tablet as normal. Skip the one you missed. Do not double up.
  • Missed a whole day or more in a row: take today's dose, but get in touch with your prescriber before you go any further, especially if you're due to move up a strength soon.

Doubling your dose doesn't just fail to "catch you up", it actively increases your risk of side effects. Nausea, constipation, diarrhoea, indigestion and stomach pain are all very common with Foundayo, meaning more than 1 in 10 people get them at normal doses. Two tablets in a day pushes your exposure well past what your body has adjusted to.

Splitting a tablet isn't a workaround either. Foundayo tablets are meant to be swallowed whole. Breaking or crushing one to create a smaller "catch-up" dose isn't how the medicine is designed to release, and it isn't covered by the licence.

Does a missed dose push back your next strength increase?#

Foundayo's dose climbs in steps, from 0.8mg up to 17.2mg, with at least 30 days on each strength before stepping up. A single missed tablet within that window doesn't reset the clock. Your 30 days are counted from when you started that strength, not from your last dose.

Where it gets more complicated is if you've missed several days, particularly close to a step-up date. Orforglipron stays in your system for a while (its levels take roughly 1 to 2 days to halve), so a single missed day still leaves most of the drug in your blood. A week off is a different matter, and levels drop much further. Going straight into a higher strength after a longer gap can mean more side effects than expected. If you've missed more than a day or two right before a planned increase, speak to our clinical team first rather than stepping up as originally planned.

Missed doses and your contraceptive pill#

Orforglipron can reduce how well oral hormonal contraceptives work for 30 days after you start Foundayo, and again for 30 days after each dose increase. If you've had a treatment gap and are restarting, that 30-day window effectively starts again from your first tablet back. Use a barrier method or a non-oral contraceptive during this time if you rely on the pill.

Making it easier to stay on track#

Most missed doses come down to routine disruption rather than forgetfulness alone. Travel, time zone changes, and side effects that make you reluctant to take the next tablet are all common and understandable reasons. None of them need to feel like a failure.

A few things that genuinely help:

  • Keep the box somewhere you'll see it daily (bedside table, next to the kettle)
  • Tie it to an existing habit, like brushing your teeth
  • Set a repeating phone alarm rather than relying on memory
  • If you're travelling, pack a few extra days' supply in your hand luggage in case of delays

Foundayo doesn't need to be taken on an empty stomach or at a specific time of day, so you have flexibility to fit it around your routine. That's different from Wegovy tablets, which need an 8-hour fast beforehand.

When to actually get in touch#

A single missed dose, or one taken a bit late, isn't something you need to report. But do get in touch with our clinical team if you've missed more than one day in a row, you're due a strength increase and have had a gap, or you're avoiding doses because of side effects. Repeated missed doses lower how much orforglipron you're actually taking over time, and that can genuinely affect your results, so it's worth flagging even if it feels minor.

See your GP or call NHS 111 the same day for persistent vomiting or diarrhoea, or any signs of dehydration. Stop taking Foundayo and seek urgent care for severe abdominal pain spreading to your back, yellowing of the skin or eyes, or sudden vision loss.
