Faulty pen policy

What to do if your Mounjaro or Wegovy pen is not working, before you ask Totiva Health for a replacement.

Contact the manufacturer first

Mounjaro and Wegovy pens work differently, so use the guide for the pen you have. If it still will not deliver a dose, the manufacturer has to log the fault before we can replace the pen or refund the order. We cannot do that from a message alone.

Try the check that matches what the pen is doing. If it still fails, stop using it and call Novo Nordisk before you contact us.

The pen will not work

  • Fit a new needle and take off both needle caps.
  • Let a pen that has come straight from the fridge reach room temperature before you inject.
  • Check the liquid is clear and colourless, and that the pen is in date.

If it still will not operate, do not keep trying. Call Novo Nordisk.

The dose does not inject

  • Hold the pen flat against the skin.
  • Press and hold the dose button until the dose counter returns to 0, then keep holding for the count in the patient leaflet.
  • If nothing happens, fit a new needle and try once more.

If there is still no injection, stop and call Novo Nordisk.

Air bubbles

A small air bubble is normal in a Wegovy pen and is safe to inject. Do not shake the pen. Do not use it if the liquid is cloudy, frothy, or has particles in it.

The pen is leaking

  • A small drop at the needle tip after use is normal.
  • Stop if liquid escapes from the pen body or the dose dial.

Do not inject with that pen again. Call Novo Nordisk.

The button feels stiff

  • Replace the needle and try once.
  • Make sure the pen is at room temperature.
  • Do not force the button.

If it stays stiff, treat the pen as faulty and call Novo Nordisk.

The dose comes out slowly

This often happens when the pen is still cold, or when the button is not held until the counter shows 0. Let it warm up and hold for the full count in the leaflet. If it keeps happening, call Novo Nordisk.

You are not sure the dose went in

  • Check the dose counter has returned to 0.
  • Look at the injection site for a large leak.
  • Do not inject another dose.

Call Novo Nordisk, then contact us before your next injection if you still are not sure.

What counts as a faulty Wegovy pen

  • The button stays jammed and will not click.
  • The dose dial spins freely or will not lock.
  • Liquid leaks from the pen body, not just the needle tip.
  • The pen is cracked, the liquid is cloudy, or parts are loose before use.
  • The injection stops halfway through the dose.

Ask for product complaints. Novo will log the issue and give you a reference number. A replacement or refund is only arranged after they confirm a device fault.

Mounjaro KwikPen troubleshooting

A KwikPen holds four weekly doses. Follow the check that matches the problem. If it still fails, call Eli Lilly before you contact us.

The pen will not start

  • Fit a new needle straight and fully, and take off both caps.
  • Hold the pen flat against the skin and press the button until you hear the first click.
  • Keep the pen in place until you hear the second click and the yellow indicator has stopped moving.

If it still does not click, fit a new needle and try once. A bent needle is a common cause. If that does not fix it, call Lilly.

You only heard one click

Each injection has two clicks: one when it starts and one when it finishes. If you lift the pen after the first click, the dose may not be complete. Hold for the full count in the patient leaflet. Do not inject again. Call Lilly if you are not sure the dose finished.

Air bubbles or the liquid looks wrong

A small air bubble is normal. Mounjaro should look colourless to slightly yellow. Do not use the pen if the liquid is cloudy, frozen, or discoloured, or if it has particles in it. Store unused pens at 2–8°C and never freeze them.

The pen is leaking

  • A small drop at the needle tip after use is normal.
  • Stop if liquid leaks from the pen body, the dose window, or around the plunger.

Do not try to repair the pen. Call Lilly.

The yellow indicator stops halfway

The yellow bar should move through the window during the injection. A cold pen can make this slower. If you held for the full count in the leaflet, the dose was still likely delivered. If the bar stops halfway and you did not hold for that count, do not inject again. Call Lilly before the next dose.

You are not sure the dose went in

  • Do not use another pen. A second injection could be too much.
  • Note the batch number, and the date and time you tried to inject.
  • Use the diagram below to see how many doses the pen has already used.

How many doses are left

Look through the small window near the base of the pen. The plunger starts at the bottom and moves up as each dose is used. When it sits at the top of the window, the pen is empty.

When a Mounjaro pen can be replaced

  • A replacement depends on how many doses you have already used successfully.
  • A pen that has already delivered 3 doses does not qualify for a replacement pen.
  • Any refund or credit is looked at case by case, and only after Lilly has confirmed a device fault.
  • If you have not had 4 successful doses from this pen, stay on your current strength until you have, before you move up.

What counts as a faulty Mounjaro pen

  • The button will not click after a new needle has been fitted.
  • Liquid leaks from the pen body, dose window, or plunger.
  • The pen is cracked, or the liquid is cloudy, frozen, or has particles in it.
  • The yellow indicator does not move and no dose is delivered.

Lilly handles KwikPen enquiries directly. You do not need to contact Totiva before you call them. A replacement or refund is only arranged after they confirm a device fault.

Email hello@totiva.co.uk or call 020 3535 8993 only after the manufacturer has logged the pen. We are open Monday to Friday, 9am–5pm. Include:

  • Your Totiva order number
  • Which pen it is, Mounjaro or Wegovy, and what happened
  • Clear photographs of the pen, the liquid, and any leak or damage
  • The batch number and expiry date
  • The reference number from Lilly or Novo Nordisk

Keep the pen. Do not post it unless we ask you to. If we do, send it by tracked post to Totiva Health, 17 Beresford Way, Chesterfield, S41 9FG, with your order number and the manufacturer reference. Once the manufacturer has confirmed a fault, we will tell you whether we can replace the pen or refund the order.

Side effects

A side effect is not the same as a faulty pen. Speak to your prescriber, and report it through the MHRA Yellow Card scheme. You can also report a side effect to the manufacturer on the numbers above.

Sharps disposal

Never put pens or needles in household waste. Used needles belong in a sharps bin. If you do not have one, contact us and we will tell you how to get one.
