Key Takeaways

Eat protein-rich, high-fibre meals in smaller portions on Mounjaro, no calorie counting needed. Manage nausea and constipation through food timing and hydration. A 7-day plan and shopping tips can make this easier.

What should I actually eat on Mounjaro?#

If you've just started Mounjaro (tirzepatide) or you're waiting on your first prescription, the food question is usually the first thing that comes up. The honest answer is that there's no special "Mounjaro diet". What works is a fairly ordinary approach: enough protein to protect your muscle, plenty of vegetables, whole grain carbs, and a sensible amount of fat, eaten in smaller portions than you're probably used to.

Mounjaro works on your GLP-1 and GIP hormones, which slows digestion and turns down hunger signals. That's genuinely useful for weight loss, but it also means you'll likely feel full much sooner than before, sometimes uncomfortably so. The food choices below are less about restriction and more about making sure that smaller appetite still delivers the nutrients your body needs.

We see a lot of patients through our weight loss service who are anxious about getting the diet side "wrong". You don't need to. You need a rough framework, a week of ideas to fall back on, and a plan for the days when nausea or fatigue makes cooking feel like too much.

The balanced plate, without calorie counting#

You don't need an app or a food scale to eat well on Mounjaro. A simple plate model does most of the work:

  • Protein, about a quarter of your plate. Chicken, turkey, fish, eggs, Greek yoghurt, tofu, or lentils. Aim for roughly 25-35g per meal, about the size of your palm.
  • Vegetables, half your plate. Anything non-starchy: broccoli, spinach, peppers, courgettes, mushrooms. The fibre here slows digestion further and helps you feel satisfied on less food.
  • Complex carbohydrates, a quarter of your plate. Brown rice, oats, sweet potato, wholegrain bread, or beans and lentils (which also count towards your protein).
  • Fat, roughly a thumb's worth. Olive oil, avocado, nuts, or oily fish.

The reason this matters more on Mounjaro than on a normal diet is that you're eating less overall. If a smaller plate is mostly refined carbs and sauce, you'll end up short on protein and fibre, and that's when people start losing muscle alongside fat, or feel constantly tired.

A 7-day meal plan you can actually use#

This isn't meant to be followed to the letter. Swap days around, repeat favourites, and adjust portions to your own appetite and injection day (many people find their appetite is lowest for 24-48 hours after dosing).

Monday Breakfast: Scrambled eggs (2) with spinach and a slice of wholegrain toast. Lunch: Tinned salmon, mixed leaves, cucumber, and a small jacket potato. Dinner: Chicken stir-fry with peppers, broccoli, and brown rice.

Tuesday Breakfast: 150g Greek yoghurt with berries and a tablespoon of ground flaxseed. Lunch: Lentil soup with a wholemeal roll. Dinner: Baked cod, roasted courgette and tomatoes, small portion of quinoa.

Wednesday Breakfast: Overnight oats made with milk, chia seeds, and sliced banana. Lunch: Leftover chicken stir-fry from Monday (batch cooking pays off here). Dinner: Turkey mince chilli with kidney beans, small portion of rice.

Thursday Breakfast: Two boiled eggs and half an avocado on toast. Lunch: Tuna and white bean salad with olive oil dressing. Dinner: Vegetable and lentil curry with a small amount of basmati rice.

Friday Breakfast: Protein smoothie: Greek yoghurt, frozen berries, spinach, milk. Lunch: Leftover curry from Thursday. Dinner: Grilled chicken breast, roasted sweet potato wedges, green beans.

Saturday Breakfast: Porridge oats with a spoon of peanut butter. Lunch: Omelette with mushrooms and cheese, side salad. Dinner: Prawn stir-fry with rice noodles and plenty of vegetables.

Sunday Breakfast: Wholegrain toast with scrambled eggs and grilled tomatoes. Lunch: Roast chicken (batch cook extra for the week), roasted vegetables, small roast potatoes. Dinner: Soup made from Sunday's leftover roast chicken and vegetables.

Snacks throughout the week, if you need them, work best small and protein-led: a boiled egg, a small pot of cottage cheese, a handful of nuts, or vegetable sticks with hummus.

Shopping on a budget#

You don't need anything specialist to eat this way. A typical week of ingredients for one person (eggs, chicken, fish, tinned pulses, frozen vegetables, oats, yoghurt, and a few fresh vegetables) usually costs somewhere between £30 and £45 at Aldi or Lidl, a bit more at Tesco or Sainsbury's depending on brands. Frozen vegetables and tinned fish and pulses are genuinely just as nutritious as fresh, and they cut down on waste when your appetite is unpredictable. Own-brand Greek yoghurt, frozen berries, and dried lentils are the easiest ways to keep costs down without compromising on protein or fibre.

Managing side effects through food#

Gastrointestinal side effects are common with tirzepatide, particularly in the first few weeks or after a dose increase. Food choices genuinely help here.

For nausea:

  • Eat smaller, more frequent meals rather than three large ones
  • Try ginger tea, which has reasonable evidence behind it for nausea
  • Cold or room-temperature foods are often better tolerated than hot, rich meals
  • Avoid very greasy or heavily spiced food when you're feeling queasy

For constipation:

  • Increase fibre gradually rather than all at once, sudden increases can make bloating worse
  • Ground flaxseed (2 tbsp gives around 3.8g fibre), berries, and leafy greens all help
  • Aim for around 2 litres of water a day
  • Gentle movement, like a short walk, also helps things along

For fatigue:

  • Keep protein intake steady across the day rather than loading it all into one meal
  • Include iron-rich foods (lean red meat, spinach, lentils) if your energy dips
  • Don't skip meals entirely, even a small nutrient-dense snack is better than nothing

Hydration and injection days#

It's worth drinking a bit more water than usual on the day you inject and the day after, since nausea and reduced appetite can lead to lower fluid intake without you noticing. Sipping water throughout the day is easier to tolerate than large amounts at once if your stomach feels unsettled.

Alcohol#

There's no rule that says you must avoid alcohol on Mounjaro, but it's worth being cautious. Alcohol can irritate the stomach lining and worsen nausea, and because Mounjaro slows gastric emptying, alcohol may be absorbed more unpredictably. Many patients also find they simply feel drunker on less than they used to. Keeping intake modest, and avoiding it entirely on days when nausea is already an issue, tends to work best.

Vegetarian, vegan, and dairy-free adjustments#

The plate model works just as well without meat or dairy. Swap animal protein for tofu, tempeh, edamame, lentils, chickpeas, or a plant protein powder if you're struggling to hit your targets from food alone. Fortified plant milks and yoghurts can replace dairy, and B12 is worth keeping an eye on if you're cutting out meat and fish, either through fortified foods or a supplement, since Mounjaro's appetite-suppressing effect can make it harder to eat a varied enough diet without a bit of planning.

Do I need to count calories?#

Not really. Mounjaro does a lot of the portion control work for you by reducing appetite. Most people find the balanced plate approach far easier to sustain than tracking numbers, and it's less likely to trigger the kind of restrictive thinking that tends to backfire once you come off the medication.

How much protein do I actually need?#

A reasonable target is 1.2 to 1.6g of protein per kilogram of body weight per day. For someone weighing 75kg, that works out to roughly 90-120g spread across the day. This matters more than people expect, because rapid weight loss on GLP-1 medications can come with a meaningful loss of lean muscle if protein intake is too low, something researchers have flagged in trial data on tirzepatide's effects on body composition.

A word on NHS access and private prescriptions#

NICE has approved tirzepatide for weight management in adults meeting certain BMI and comorbidity criteria, but NHS access is currently limited and rolling out gradually through GP practices and specialist weight management services. Many people in the UK access Mounjaro privately in the meantime. Wherever you get your prescription, it should come with proper clinical oversight, including a review of your suitability and ongoing monitoring, not just a form to fill in.

At Totiva, weight loss treatments including Mounjaro require a free online consultation, followed by a short video call with a clinician before your first prescription, which is a regulatory requirement for GLP-1 medications. It's quick (usually 10-15 minutes), and you only pay if you're approved and choose to go ahead. Current pricing for Mounjaro through Totiva ranges from £136.88 for the 2.5mg starting dose up to £268.88 for the 15mg dose, depending on where you are in your treatment.

Take home message#

There's no perfect Mounjaro diet, just a sensible one: enough protein to protect your muscle, plenty of vegetables and fibre, whole grain carbs, and portions that match your reduced appetite rather than fighting it. Pay attention to how different foods sit with you, adjust as your side effects settle, and don't be afraid to keep meals simple on the days when cooking feels like too much.

The habits you build now, eating enough protein, staying hydrated, not skipping meals entirely, are the ones that will help you keep the weight off long after you've finished treatment. If you'd like to talk through whether Mounjaro is right for you, Totiva's pharmacist-led consultations are a straightforward way to get a clinical opinion without any pressure to commit.
