Does Scalp Massage Help Hair Growth? A Small Benefit at Best

Scalp massage is low-risk, but the evidence is tiny: one small trial in healthy men without hair loss. It does not treat pattern hair loss. Here are the numbers and a simple routine.

Key Takeaways

Scalp massage is low-risk if you are gentle, but the evidence is very weak. The one small trial was in healthy men without hair loss and measured hair thickness only, so it is unproven for thinning hair. It does not treat pattern hair loss or alopecia areata. If you try it, be consistent for about 6 months, and don't delay proven treatment.

Scalp massage is cheap, pleasant and low-risk if you are gentle, but it is not a proven hair loss treatment. The one small trial that measured a change found a slight increase in hair thickness in healthy men without hair loss, so we do not know whether it helps thinning hair. If you have pattern thinning, treat massage as a harmless extra, not a replacement for a licensed treatment.

The claim#

The idea is simple. Massage is said to increase blood flow to the scalp and stretch the cells at the base of each follicle (the dermal papilla cells), which help control hair growth. Some people also hope it lowers stress, which can trigger shedding.

All of that sounds sensible. Sounding sensible is not the same as being tested.

What the studies found#

The study people quote most is a 2016 Japanese trial of 9 healthy men who did not have hair loss. They followed a standardised scalp massage protocol using a massage device for 4 minutes a day for 24 weeks. Average hair thickness rose slightly by the end. The trial measured thickness only, not hair density or hair counts. There was no control group, and the group was far too small to prove anything, especially for people who are losing hair.

A 2019 survey asked people with androgenetic alopecia (pattern hair loss) about their own massage routines. Many who kept going for months reported less shedding or some regrowth. But the results were self-reported, nobody measured hair, and people who did well were more likely to reply.

Both are weak sources. Neither is strong enough to show that massage regrows hair, and the sources used for this article include no large randomised trial.

Which kinds of hair loss it might suit#

  • Pattern hair loss: this is driven by follicles being sensitive to the hormone DHT. Massage does not change that, so it cannot be expected to halt it.
  • Stress-related shedding (telogen effluvium): this usually settles by itself within about 3 to 6 months once the trigger passes. Massage may help you relax, but it is not what regrows the hair.
  • Alopecia areata: this is an autoimmune condition causing smooth, sudden patches. Massage will not treat it.
  • Scarring alopecias: these destroy the follicle, so massage has nothing to work on.

A fair way to try it#

Use your fingertips, not your nails. Press firmly enough to move the scalp over the skull rather than sliding over the hair. Work in small circles across the whole scalp, once a day. A few minutes is a reasonable session. The 2016 trial used 4 minutes a day, but it used a standardised device, so fingertip massage may not apply the same stretching force and may not give the same result. Oil is optional, and there is no evidence that oils add benefit. A handheld massager is fine too.

Take a photo in the same light on the same day each month. Judge the result after 6 months, not 6 weeks.

Skip it if your scalp is sore, flaky or inflamed, for example with seborrhoeic dermatitis, psoriasis or folliculitis. Never tug or pull. Repeated tension can cause traction hair loss.

Will it make more hair fall out?#

You may see a few hairs on your fingers. Everyone sheds hairs every day, and some of those may come out during massage. Keep the pressure gentle and stop if your scalp becomes sore. If you notice more shedding than usual, or it carries on, see your GP.

Using it with minoxidil or finasteride#

Massage can sit alongside either one. There is no proof that it helps minoxidil absorb better. Massage first, then apply minoxidil to a dry scalp and wash your hands afterwards. Hard rubbing on freshly treated skin may cause irritation.

Finasteride 1mg is prescription-only in the UK and licensed for men. The MHRA has updated its warnings about mood changes and sexual side effects, and patients are given an alert card. Anyone can report a suspected side effect through the Yellow Card scheme. Totiva's hair loss service is for men only. Women with thinning hair should see their GP, who can advise on options such as topical minoxidil.

Verdict, and when massage is the wrong tool#

Try it if you enjoy it and can stick with it, but don't expect it to treat hair loss. Do not wait six months if something else is going on. See your GP if hair loss is sudden, patchy, painful, scaly or scarring, or comes with tiredness, weight change, heavy periods or thyroid symptoms. You can ask about blood tests for ferritin (iron stores), thyroid function, vitamin D and B12. The NHS does not usually prescribe finasteride or minoxidil for male pattern baldness, so most people buy these privately.

If you want to compare the treatments that do have trial evidence, start with finasteride vs minoxidil.
