Does a Calorie Deficit Work in Perimenopause? Yes, but Go Gently

A calorie deficit still works in perimenopause, but a smaller cut and some muscle protection matter more than before. Here are the numbers to aim for.

Key Takeaways

Yes, a calorie deficit still works in perimenopause. Aim for a modest cut of 300 to 500 kcal a day, eat enough protein and lift weights twice a week to protect muscle.

Yes, it does. Eat less than you burn and you will lose weight, in perimenopause as at any other age. What changes is how easy it feels, and how much muscle you lose if you cut too hard.

Why it feels harder than it used to#

The US SWAN study followed women through the menopause transition. It found that fat mass rose and lean mass fell, and the fat tended to settle around the middle. That is why your waist can grow when the scales barely move.

Menopause itself explains only a small part of the drop in the calories you burn. Most of it comes from ageing, losing muscle and moving less. Broken sleep and night sweats add to the problem, because tired people feel hungrier and reach for snacks.

So your maintenance level may be somewhat lower than it was ten years ago. It is not broken. The same approach just needs a lighter touch.

A realistic deficit#

The NHS uses about 2,000 kcal a day as a guide for the average woman. Your own figure depends on your size and activity.

If 2,000 kcal is roughly your maintenance, a cut of 300 to 500 kcal a day gives a target of 1,500 to 1,700 kcal. That usually means losing 0.25 to 0.5kg a week. Over 12 weeks, expect around 3 to 5kg (about 7 to 11lb).

The NHS says 0.5 to 1kg a week is a safe upper limit. In perimenopause, the slower end is more realistic and easier to keep up.

Crash dieting is a poor idea at this stage. Very low calorie diets (under about 800 kcal) should only be used under specialist supervision, as NICE advises. Cutting too hard speeds up muscle loss, and your bones are already losing some protection as oestrogen falls.

Protect your muscle while you cut#

Muscle is what keeps your calorie burn up, so it is worth guarding.

  • Protein: include a good source of protein at each of your 3 meals, and do not let it drop as you cut calories. Eggs, fish, Greek yoghurt, beans and chicken all work. Our high-protein, low-calorie foods list has ideas. If you have a medical condition that affects your diet, check your protein needs with your GP or a dietitian.
  • Strength work: do muscle-strengthening exercise at least 2 days a week, as the UK Chief Medical Officers advise. Try 2 to 3 sets of 8 to 12 reps of squats, rows and presses, using weights that feel hard by the last few reps.
  • Walking: daily steps help, but they do not replace strength work.
  • Bones: you need 700mg of calcium a day from food such as dairy, fortified plant milks or tinned fish with bones. The NHS also advises 10 micrograms of vitamin D daily in autumn and winter.
  • Alcohol: keep to the UK limit of 14 units a week or less. It adds calories and worsens sleep and hot flushes.

Where HRT fits#

NICE guideline NG23 supports offering HRT for menopause symptoms such as hot flushes, low mood and poor sleep. HRT is not licensed as a weight-loss treatment, and it will not do the work of a calorie deficit.

It may help indirectly. If it settles your night sweats and sleep, healthy eating and exercise can feel easier. Some studies suggest small benefits for where fat is stored, but the evidence is modest. HRT has risks as well as benefits, so it is worth discussing with a GP or menopause specialist.

If the scale will not move#

Check your waist as well as your weight. The NHS says a waist of 80cm (31.5in) or more in women raises health risk, and 88cm (34.5in) or more raises it further. Waist size can shrink while weight stays put, especially once you start lifting.

Give any change at least 4 to 6 weeks. Check that your portions and tracking are accurate, and look at your sleep. Our guide on why you are not losing weight in a calorie deficit goes through the common causes.

See your GP if your weight changes quickly or you feel very tired, cold or low, as a thyroid check may be needed. If diet and exercise alone are not getting you where you want to be, come to us at Totiva. Medicines are an option for some people alongside diet and exercise, and we can talk through whether they might suit you.
