In the SURMOUNT-1 trial, around 4% of participants taking tirzepatide reported alopecia, compared to 0.7% in the placebo group. The MHRA classifies 'common' side effects as those affecting between 1 in 10 and 1 in 100 people, so the real-world figure may sit higher than the trial data alone suggests.

This is an important distinction. Clinical trials use highly controlled populations. In the broader real-world setting, where people are losing weight faster, eating less consistently, or dealing with more stress, the incidence of hair shedding may be higher than 4%.

It's also worth noting that the label says hair loss 'can affect up to 1 in 10' people, which reflects the upper bound of the common classification. So the range is quite wide, and individual experience varies considerably.

Why Does Mounjaro Cause Hair Thinning?#

Here's where it gets nuanced. It's not entirely clear whether the hair loss is a direct effect of tirzepatide on hair follicles, or whether it's an indirect consequence of the rapid and significant weight loss the drug produces. Probably both are playing a role in different people.

The main mechanism is something called telogen effluvium. This is a well-recognised condition where a physical or metabolic stressor causes a large number of hair follicles to simultaneously shift into the resting (telogen) phase of the hair growth cycle. Normally, only about 10-15% of your follicles are resting at any one time. In telogen effluvium, that proportion spikes dramatically, and around two to three months later, those resting hairs shed all at once.

The typical triggers include surgery, serious illness, childbirth, crash dieting, and, notably, rapid weight loss. Research has shown that losing weight faster than the recommended 0.5-1kg per week significantly increases the risk of hair shedding compared to losing weight more gradually. Given that Mounjaro produces an average of around 21% body weight reduction over 18 months, the pace of loss, particularly in the early months, can be substantial.

One key question is whether there's something specific to tirzepatide's mechanism (its dual GIP and GLP-1 receptor agonism) that independently disrupts follicle cycling. At this point, there's no confirmed evidence that it does, but research is ongoing.

Does Hair Loss Get Worse at Higher Mounjaro Doses?#

This is something many people ask, and the honest answer is that the available data doesn't clearly separate hair loss rates across the 2.5mg to 15mg dose escalation range. The SURMOUNT-1 trial combined data across doses. What we can reasonably infer is that weight loss accelerates with higher doses, and since more rapid weight loss is a known driver of telogen effluvium, there may be an indirect dose-related effect. If hair shedding increases as your dose goes up, it's worth mentioning to your prescriber.

How Does Mounjaro Compare to Wegovy for Hair Loss?#

Semaglutide (Wegovy/Ozempic) is similarly associated with hair loss. In the STEP 1 trial for semaglutide, alopecia was reported in approximately 3% of participants. So the rates are broadly similar to Mounjaro, which makes sense given that both drugs produce significant weight loss through appetite suppression. The hair loss seen with both medications is almost certainly driven predominantly by the weight loss itself rather than by any unique pharmacological property of either drug.

When Does Hair Loss Start and Stop?#

Telogen effluvium follows a fairly predictable timeline once you understand the biology. The follicle stress occurs first, usually in the first few weeks to months of starting Mounjaro or during the period of most rapid weight loss. The shedding then happens roughly 2-3 months after that trigger. So many people notice hair loss around 3-4 months into treatment, sometimes later.

The shedding phase itself typically lasts 3-6 months. Once the triggering stressor has passed (i.e. your weight loss slows and your body adapts to the lower intake), hair follicles start cycling back to normal, and regrowth begins. For most people, hair returns to its previous thickness within 6-12 months of the shedding starting.

This is broadly reassuring, but it doesn't make the experience any less distressing while it's happening, particularly for women where diffuse thinning is more visible.

How to Protect Your Hair on Mounjaro#

There are practical steps that can reduce the severity of hair shedding, even if they can't guarantee you'll avoid it entirely.

Eat Enough Protein#

Hair is made predominantly of keratin, a protein. When total calorie and protein intake drops sharply, the body de-prioritises non-essential processes, and hair growth is near the top of that list. Mounjaro significantly suppresses appetite, which is the point, but this can lead to protein intakes falling well below what the body needs.

Aim for at least 1.2-1.6g of protein per kilogram of body weight per day. For most adults on Mounjaro, this means actively prioritising protein at every meal rather than relying on appetite cues alone. Eggs, chicken, fish, Greek yoghurt, lentils, and cottage cheese are all good options.

Don't Drop Calories Too Fast#

Extremely low calorie intake amplifies the metabolic stress that drives telogen effluvium. Mounjaro makes it easy to eat very little, but trying to stay above 1,200 kcal per day (or following your clinician's guidance on minimum intake) reduces the shock to your system.

Iron, Zinc, and B Vitamins Matter#

Research consistently shows that deficiencies in iron, zinc, biotin, and folate contribute to hair shedding and poor regrowth. These deficiencies become more common when overall food intake drops. A good quality multivitamin or targeted supplementation can help, though it's worth checking your iron levels with a GP before supplementing iron specifically, as too much can cause problems.

Biotin supplements are widely marketed for hair health. The evidence for biotin improving hair in people who aren't deficient is actually thin, but deficiency is common in people eating restricted diets, so supplementing at a standard dose is unlikely to do harm.

Manage Stress Where You Can#

Psychological stress has its own independent effect on hair follicle cycling, separate from nutritional stress. Major life stressors compound the telogen effluvium risk. Addressing sleep, stress levels, and wellbeing isn't just good advice in general, it genuinely does affect hair health.

When to Consider Topical Treatments#

For people experiencing significant shedding, or for men who have a background of androgenetic alopecia (male pattern baldness) that may be worsening alongside the telogen effluvium, topical minoxidil is worth considering. It works by prolonging the growth phase of hair follicles and increasing blood flow to the scalp.

Topical minoxidil won't address the underlying cause of Mounjaro-related hair loss, but it may help reduce visible thinning and support earlier regrowth. Red Flags: When Hair Loss Needs Urgent Attention#

Telogen effluvium is diffuse, meaning hair sheds fairly evenly across the scalp rather than in patches. If your hair loss looks different, it may not be telogen effluvium and may need prompt assessment.

See your GP sooner if you notice:

  • Patchy hair loss (smooth circular bald patches suggest alopecia areata)
  • Hair loss with scalp redness, scaling, or itching (could indicate a fungal infection or inflammatory condition)
  • Eyebrow or eyelash loss alongside scalp shedding
  • Hair loss accompanied by significant fatigue, weight gain (rather than loss), or cold intolerance (could suggest thyroid issues)

Should You Stop Mounjaro Because of Hair Loss?#

For the vast majority of people, stopping Mounjaro solely because of hair loss isn't the right call. The condition is almost always temporary, the shedding typically resolves within months, and the health benefits of sustained weight loss are significant. But this is a personal decision that should involve your prescriber, who can weigh up the severity of the hair loss against your weight loss goals and overall health.

What About Acupuncture for Hair Loss?#

With the evidence for Mounjaro-related hair loss pointing firmly towards a nutritional and metabolic cause, it's worth briefly addressing acupuncture, which is increasingly searched as an alternative approach.

Some small studies have suggested that acupuncture may modestly improve scalp blood flow and have anti-inflammatory effects relevant to conditions like alopecia areata.
