Key Takeaways

Unopened Mounjaro pens last until their printed expiry date when kept in the fridge at 2°C to 8°C. Once you inject your first dose, discard the pen after 30 days, even if doses remain.

Unopened Mounjaro KwikPens are stable until the expiry date printed on the carton and pen label, provided they are kept refrigerated at 2°C to 8°C. Once you use your first dose from a pen, you have 30 days to use the remaining doses before the pen must be discarded, regardless of how many doses remain.

How the 30-day rule works#

The 30-day clock starts at first use, not when you take the pen out of the fridge. An unopened pen can be removed from the fridge, kept at room temperature not exceeding 30°C, and returned to the fridge again without starting the countdown. The clock only begins when you inject your first dose.

After first use, you can keep the pen either in the fridge (2°C to 8°C) or at room temperature, as long as that temperature stays below 30°C. After 30 days from first use, discard the pen even if doses remain.

Each KwikPen contains four weekly doses. On the standard once-weekly schedule, four doses take four weeks, so the 30-day limit and the dose count align neatly. Mark the date of first use on the pen label so you do not have to rely on memory.

Fridge storage: what actually matters#

Store unopened pens in the main body of the fridge, not in the door. The door is the warmest part and experiences the most temperature variation every time it is opened. Keep pens away from the freezer compartment too: freezing permanently damages the medication and a frozen pen must be discarded.

If you receive several pens at once (which can happen with private prescriptions or when an NHS batch is dispensed together), store all unused pens in the fridge until you need them. Do not leave spare pens at room temperature to "get started" early.

Older or poorly calibrated fridges can run warmer or colder than expected. If you are unsure whether your fridge stays consistently between 2°C and 8°C, a small fridge thermometer costing a few pounds will confirm it.

What happens above 30°C#

Tirzepatide, the active ingredient in Mounjaro, is a synthetic chain of amino acids. Heat disrupts the bonds holding that structure together. Above 30°C, the molecule can begin to degrade, reducing the amount of active drug delivered with each dose. You would not necessarily be able to tell from looking at the pen.

This matters particularly during UK summer heatwaves. Many UK homes lack air conditioning, and indoor temperatures can exceed 30°C for days at a time. If your home is regularly above 30°C, keep the in-use pen in the fridge between doses rather than at room temperature. Take it out 30 minutes before injecting so it reaches room temperature, which makes the injection more comfortable.

How to check the pen before each dose#

Before every injection, inspect the solution through the pen window. Normal Mounjaro is clear and colourless or very slightly yellow.

Discard the pen if you see any of these signs:

  • Cloudiness or a milky appearance
  • Visible particles or floating matter
  • Significant yellowing or other discolouration
  • Crystals on the inside of the pen

Do not inject from a pen that looks abnormal, even if it is within date and has been stored correctly.

If you suspect you have already injected from a degraded pen and you notice unexpected changes in appetite control, report it via the MHRA Yellow Card scheme at yellowcard.mhra.gov.uk. Mounjaro carries a black triangle (▼) in the UK, meaning enhanced post-market safety monitoring is ongoing and patient reports are genuinely useful.

Travelling with Mounjaro#

Keeping the pen cool while travelling takes a little planning. FRIO cooling wallets use evaporative technology to maintain a safe temperature range without ice. Check the manufacturer's guidance for cooling duration and performance under the conditions of your specific trip, as this varies with ambient temperature and humidity.

For air travel, injectable prescription medicines are generally exempt from standard liquid restrictions, but rules vary by airline and airport. Carry a letter from your prescriber confirming the medication is clinically necessary, keep the pen in its original packaging, and check with your airline and the relevant airport authority before you travel.

Do not pack the pen in checked luggage. Aircraft hold temperatures can drop well below freezing.

Disposing of expired or unusable pens#

Do not put used or expired Mounjaro pens in household recycling or general waste. The needle is a separate consumable that you attach before each injection, and both the used needle and the pen body must be disposed of in a sharps bin. Used pens and needles should go into a sharps bin — your GP surgery or local council may provide these. For unwanted or expired medicines that have never been used, the Community Pharmacy Medicines Disposal scheme allows you to return them to a local pharmacy. Check with your local pharmacy about what they accept, as rules differ in Scotland, Wales and Northern Ireland.

Your pharmacist can advise on the nearest sharps disposal option if you are unsure.

Reading the expiry date#

The expiry date on the Mounjaro pen and carton appears after the letters EXP and is written as month and year (for example, 06/2026). This refers to the last day of that month. A pen marked 06/2026 is usable up to and including 30 June 2026, provided it has been stored correctly. Never use a pen past this date.
